Replace the clamp with a screw type hose clamp. Here is an example Northern Industrial Zinc-Plated Hose Clamps — 34-Pc. Set | Hardware Kits | Northern Tool + Equipment

Without being there I cannot say what the problem is. Sounds like the connector to the faucet adapter is either worn out or broken.
