The top spray arm fell off and it's not clear how to replace it. I dread having to remove the unit (it was a bear to install because of how the previous homeowner installed the tile floor). Any chance it can be repaired in-place?

Worth a try
Depends what broke/stripped, if any thing.
Also if the gasket on the outside of the tub has not moved.
Be sure to clean/check the holes in the spray arm before reinstalling.
It's rotation direction should have tended to keep it on, so perhaps it was not turning.
Basically you will not know till you try it.
Do not over tighten it.
