The upper spray arm fell off during wash cycle. It looked fine but I purchased a new one and snapped it into place. The new one came off during the next wash cycle. I then purchased a new spinner nut and snapped the new spray arm in place. It came off again during the second wash cycle. Is the upper spray arm held in place by only the spinner nut? Is there anything that can hold it more securely?

UPDATE: OK, I'm not being clear enough. The spinner nut stays on the upper hose. It tightens securely and the upper hose threads are fine. The problem is with the upper spray arm coming off of the spinner nut. Is there any way to spread the "clips" on the upper spray arm to make it stay in the spinner nut?

Looks like the threads on the upper part of the upper spray arm hose (#9 at the diagram) is damaged. You have to order it together with the washer (#11). The dishwasher has to be removed from the cabinet to replace this hose.
