First place to check is the electric connection. It will be protected by a metal box, whose lid is usually secured with one screw (normally 1/4 inch hex drive).I have seen a LOT of installers merely wire-nut the wires together. They should be as securely twisted together as you can get them, wire-nutted, and then taped, so no movement can pull a wire free.
