If there is no broken or loose wire, then the optic boards are the first failure suspect.

Gene, where would I look for a loose/broken wire? The wire harness coming out of the wall to the icemaker is about 6-8" long. Is it possible that it could be loose/broken somewhere in the wall that it goes into?

I am about to buy the optics kit, but wanted to check for this first.

As it shown in the wiring diagram, you can check for 120 VAC between the BK/WH 4 and WH 8 at the receiver board.

If there is no proper voltage, then the boards are the culprit.

OK, it's definitely the board as you can see from the attached image... I pulled it out of the frame and there was a nickel-sized burn mark on the frame wall, along with a spot on the board that was charred as well (just above the screw).

What could have caused this?

I'll order the new kit and post the results asap.
