I need to replace the glass cooktop. I don't know how to remove it and need the part number. Do I need to replace any other parts (gaskets, etc) when replacing the glass?

open the oven door, in the front under the cooktop are 2 screws, remove the screws....Don't forget to unplug the oven. next lift the front of the cooktop up. depending on the age, there should be molex plugs connected to the rear of the oven, you will see them once you lift the cooktop up. dissconnect the plugs. Now the cooktop should lift of the pegs in the rear, you will have to wiggle a bit, but it will come off. all that's left to do is transfer the burners to the new cooktop and install.
