The washer was making a lot of noise for awhile before the washer stopped spinning. So I took it apart to find the belt off and the large pulley to be loose. Next I tightened the wheel and put the belt back on. Well a few weeks later the same thing has happened. My question is why does the pulley keep loosening? Could the belt be stretched and in need of a new one?

I don't think the issue is the belt, I think the pulley is loosening and that is causing the belt to come off.

The pulley nut may be stripped or the shaft the nut goes on may be stripped. Examine both and hope it is the nut. Make sure you are using a 21 mm wrench to tighten the nut.
