The washer agitates fine, but when it comes to the rinse and spin cycle, it doesn't always spin. If it doesn't spin, it will be making noises like it is trying to spin. Sometimes if I lift the lid and close again, it may or may not spin. What could be causing this?

The clutch is likely slipping. See video on how to replace. Eric

Thanks. Do I likely need the "clutch assembly" or the "clutch band & lining kit"?

Depends on the condition of your clutch hub. $12 difference in price.
