The ice at the top of the door could be caused by a leaky door gasket. Check the seal with a dollar bill. The gasket should hold the dollar snugly. The excess moisture would also cause more ice buildup on the evaporator coil behind the back panel inside the freezer which could also be caused by a defrost system problem. Check the coils.
