need jennair vent termination

Model Number: ?????? Brand: Jenn-Air Age: Less than 1 year

I'm looking for a new 5" out door vent termination for a Jennair down draft range. This is plastic and has a positive close on flapper. It says Jennair on the outer part of the hood. Where do I find this accessory?
