This might help you find the Model Number

Did find one thread with Gene on a Performa range, the Model number given was PER5750QCW.
Use the Advanced Search for Performa in the Oven Repair forum only to see them.

Here is a service manual and a tech sheet for the PER5750QCW

When you say lost what exactly do you mean?
Does the stove top run OK?
Does the oven light work?
Try flipping the breaker off/on slowly a couple times, could be you have lost half the line, sometimes this can happen without actually tripping the breaker.
